DUMC Preschool Registration Forms (Print & Return)

Registration is accepted by mail or online.

To register and pay by check, fill out the above Registration Application and mail it to The Preschool at DUMC with your check payment.

To register and pay by credit card, fill out the Registration Application and email it to admin@dumcpreschool.org. You will receive an invoice by email. Deposit payments can be made by bank check or credit card online. Please note credit card processing fees will be applied.

To secure your child’s enrollment, payments must include the registration fee and one month’s tuition. Payments must be received within five days.
